How to submit a takedown notice
A valid notice must include the following:

3) A direct link (URL) that lets us locate the complained-of content on the site.
4) Your contact information (name, email) so we can reach you.
5) A statement that you have a good-faith belief that the complained-of use is not authorized by the copyright owner, its agent or the law.
6) A statement, under penalty of perjury, that the information in the notice is accurate and that you are authorized to act on behalf of the copyright owner.

Counter-notice
If you believe your content was removed by mistake, you may send a counter-notice to the same address, including your identity and a good-faith statement that you have the right to publish the content.
